LEWISBURG – Heritage Springs Memory Care in Lewisburg has lost its certificate of compliance from the state. The Daily Item reports the state Department of Human Services revoked the certificate following five new violations in its latest inspection report.
The Daily Item says the violations were not having enough staff on specific shifts, paperwork not properly completed, a new resident didn’t have a cognitive screening in a timely manner and a threat from one resident to another went unreported. The violations stem from inspections in April, May, and June.
The Daily Item says Heritage Springs also had 24 previous violations from April, February, and December.
The Daily Item says the department will conduct an on-site inspection at the mandated correction date within 20 calendar days of August 4.
In addition, The Daily Item says two Heritage Springs employees were accused last month of abusing elderly residents between December and April; They are 18-year-old Madison Cox of Montgomery and a 17-year-old male juvenile.
The Daily Item says they are accused of posing with patients in the shower or on the toilet, took pictures of patients who had defecated themselves or had fallen to the ground, and took videos of themselves demeaning or harassing individuals.
The Daily Item says Cox has been charged with 17 counts of misdemeanor abuse of a care-dependent person. She has a preliminary hearing scheduled for August 24.
The Daily Item says the five new violations are not related to the criminal investigation or accusations.
